Free! External Floppy Controller

I found this in my storage chaos and I don't remember why I kept it for so long.  I believe I tore it out of an external floppy enclosure for very good reasons...  But what those reasons are elude me.  Probably because this was smaller and easier to store. 

Anyway, as far as I know it works.  It takes 5 and 12 Volts, and connects a Centronics 50 to a 34-pin floppy drive.
